Let me start with the Pole Mounted Transformer. As the name kind of gives away, these get mounted on poles. The high voltage line comes down from overhead power lines and connects to the transformer's high voltage side. Then the low voltage side sends power out to distribution equipment, which finally gets electricity to individual homes.
These are set up on a concrete pad. The incoming cables come from underground - the high voltage line surfaces from below the ground and connects to the bottom of the transformer. Same deal on the output side: low voltage cables exit from the bottom and run to the designated distribution equipment, which then distributes power to homes.
